This program helps individuals with a doctor diagnosed chronic disease that cannot afford the medications they need. They help
low income and disabled individuals to access more than 1,200 name brand medicines for free or at low cost, including diabetic
supplies.
For Open Enrollment (October 15 – December 7), make an appointment to let us help you find the best Part D plan for you.
You would need to bring your Medicare Card and a list of your current medications with dosage and strength.
Last year, we helped DeKalb residents obtain over $3.2 million worth of their prescription medications!
